Private Duty Nurse (RN/LPN) – 1:1 Home Care

Job Summary

• Centria Healthcare is hiring dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide continuous, high-acuity 1:1 private duty nursing for a client in Wellston, MI.

  This client is authorized for 24/7 one-on-one care, typical of catastrophic injury recovery (such as Motor Vehicle Accident/MVA care). You will play a critical role in providing comprehensive, skilled clinical care in a comfortable home environment while working closely with the client, their family, and an interdisciplinary care team.

  Responsibilities

  • Provide continuous, high-acuity 1:1 skilled nursing care tailored to the client's complex medical needs.

  • Manage advanced clinical care needs, which may include tracheostomy, ventilator support, feeding tubes, wound care, and medication administration.

  • Perform thorough physical assessments, monitor daily vital signs, and track changes in condition.

  • Respond promptly and effectively to subtle changes in health status or clinical emergencies.

  • Assist with activities of daily living (ADLs) and promote patient comfort, dignity, and mobility.

  • Maintain accurate, detailed, and timely electronic charting and documentation.

  • Collaborate with family members, physicians, and Centria’s clinical leadership team to optimize outcomes.

  Shift Details & Schedule

  • Location: Wellston, MI

  • Shifts: 12-Hour Shifts (Day Shifts & Night Shifts Available)

  • Coverage Need: 24/7 Continuous Care Authorization (Full-Time, Part-Time, and PRN opportunities)

  • Pay: Weekly pay + optional daily pay; competitive hourly rates based on experience and shift differentials.

Qualifications

• Active RN or LPN license in the state of Michigan
• Home care or private duty experience preferred (not required)
• Trach/vent experience a plus (training provided)
• Strong communication and critical thinking skills

Patient Population

• Pediatrics (birth–21 years)
• Adults with complex medical needs
