Primary Care Physician (PCP) Responsibilities Independently provide comprehensive care for older adult patients with acute and chronic illnesses. Maintain full accountability for patient care and outcomes, seek specialist consultation as needed. Engage with hospitalists for admitted patients and coordinate care across inpatient, SNF, LTC, and home settings. Lead a care team consisting of a medical assistant, care facilitator, and care coordinator for office‑based patients. Participate in transitional care teams and collaborate with case managers, acute and transitional‑care physicians, and other resources. Assist in center management, cover for other providers, and help recruit patients and additional providers. Supervise, collaborate, or participate with an APP, remaining accountable for the APP’s actions while employed. Perform other duties as assigned and modified by manager. Education & Experience MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required. State medical license in the state of work; eligible for or board‑certified in chosen specialty; must maintain certification via MOC, CME, or exam re‑certification. Current DEA registration for schedule II–V controlled substances. Basic Life Support (BLS) certification from American Heart Association or American Red Cross within first 90 days of employment. Board eligibility required; board certification preferred. Compensation Pay range: $221,141–$315,915 (base hourly rate or full‑time salary, dependent on experience, education, location, etc.).
