Completion of ACGME/AOA, approved 3-year residency program in EM/FM or IM. ECFMG required for foreign medical graduates. Clinical Experience New graduates acceptable: EM residency graduates within the past 12 months may be considered. 3-month proctorship required. License Active unrestricted VA Medical License and DEA w/ VA address & schedules 2,2N,3,3N,4 & 5 Board Certification American Board of Emergency Medicine (ABEM), American Osteopathic Board of Emergency Medicine (AOBEM), American Board of Medical Specialties (ABMS) American Board of Medical Specialties FM, IM, EM. Merit Badge ACLS, ATLS and PALS if not BC in EM or EM trained. NRP for non-ABEM providers. Immunizations MMR/Varicella (2) Vaccines or titers with values indicating immunity for MMR & Varicella. TB (past 12 months) or a QuantiFERON Gold indicating immunity within three months. 2nd TB test to be completed prior to activation and privileges given. Chest x-ray required within 3 months if positive for TB or positive QuantiFERON. FLU within current season. Case Log Case Logs for the last two years, in addition any documentation for special training for Special Procedures.
