Primary Care Physician (PCP) The Primary Care Physician (PCP) provides comprehensive primary care to seniors with a focus on outcomes, value, and patient-centered care. The PCP works independently, managing a panel of up to 450 patients and collaborating closely with care teams, hospitalists, and other specialists. Responsibilities Provide independent clinical care, including assessment, diagnosis, treatment, education, health promotion, and care coordination for acute and chronic illnesses. Maintain accountability for patient outcomes, including admissions, clinical gaps, cost measures, and patient experience scores (CGCAHPS). Work effectively with hospitalists, SNF, LTC, or homebound providers, and transitional care teams as needed. Lead and coach a multidisciplinary care team (medical assistant, facilitator, coordinator). Participate in risk, quality, and compliance programs; attend clinical meetings and ad‑hoc sessions. Support center operations, including coverage for other providers, recruiting patients, and new provider onboarding. Qualifications & Skills Medical degree with board eligibility; completed residency in internal medicine, family medicine, or geriatrics. Strong clinical knowledge, evidence‑based practice, and up‑to‑date training in lifestyle and nutrition interventions. Excellent communication, leadership, and coaching ability. Experience with value‑based, outcomes‑focused practice and performance metrics. Proficiency in Microsoft Office and electronic health record systems. Ability to travel locally or regionally for training and recruitment as required. Metrics & Performance Goals Patient admissions per thousand patients. Utilization of 18‑21 appointment slots per day (new patients: 2 slots, follow‑ups: 1 slot). CGCAHPS, clinical gap closures, and medical cost measures. Compensation & Benefits Pay range: $221,141 – $315,915. Final compensation depends on experience, education, geography, and other factors; potential bonuses and commissions.
