Primary Care Physician Profile Accountability for outcomes: Provide strong clinical care and cost‑effective treatment for up to 450 patients, building trust and influencing outcomes through behavior change. Coaching for health: Act as a health coach, helping patients set short‑ and long‑term goals, partnering with them, and following up to improve health. Simplifying for action: Prioritize and simplify care plans, making behavior change actionable for patients and leading care teams toward performance goals. Responsibilities Provide independent care for acute and chronic conditions in older adults. Take full accountability for patient care and outcomes, seek specialist consultation when needed while staying involved in detailed care. Engage with hospitalists for patients admitted to the hospital, regardless of hospital affiliation. Assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with complex chronic needs. Lead the care team (medical assistant, care facilitator, care coordinator) for office‑visiting patients. Coordinate transitional care with case managers, acute and transitional‑care physicians, and other resources for patients unable to visit the office. Actively manage center operations, cover other providers when needed, and participate in recruiting patients and providers. Perform other duties as assigned and modified at the manager’s discretion. Self‑Care: Maintain physical, mental, social, and spiritual well‑being to provide best patient care. Proficiency in Microsoft Office Suite and other business software. Willingness to travel locally for practice needs and nationally for initial training and regional recruiting. Fluency in English, written and verbal; additional language fluency may be required. Independent judgment in clinical and operational decisions. Education and Experience Criteria MD or DO in Internal Medicine, Family Medicine, Geriatrics, or related specialty required. Active state medical license for the state(s) of employment. Board certification (or board eligibility) in one of the specialty areas preferred. Maintain board certification through required MOC, CME, or retesting. Current DEA number for schedule II‑V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within first 90 days of employment.
