Location and Opportunity Location: 454 St Michaels Drive, Santa Fe, NM 87505. Full‑time, FTE 1.00. Overview Provider cares for patients by providing outpatient primary care services. The provider will lead and work collaboratively with a team, including nurses, medical assistants, case and care managers, behavioral health clinicians, pharmacist clinicians, and clerical staff to provide care to patients. Practice Highlights 16‑18 patients per day 34 hours of direct patient care, 6 hours of administrative time Panel of patients Support team: Clinical Pharmacist, Behavioral Health Clinician, Care Manager, RN/MA, Physicians Compensation based on experience with incentives based on production, panel size, and quality metrics Responsibilities Provide comprehensive medical care to patients with acute and chronic illnesses. Make professional medical judgments in the care of patients seen. Order, conduct, and interpret appropriate diagnostic x‑ray and lab tests. Prescribe treatments, medications, and non‑pharmacologic therapies. Document all patient encounters and communications appropriately. Manage the provider/patient relationship. Receive and resolve patient complaints. Educate and counsel individuals, families, and groups. Serve as a health‑care resource, interdisciplinary consultant, and patient advocate. Work autonomously as well as in collaboration with other health‑care providers and staff as needed. Assist in the recruitment and replacement of other providers. Participate in system‑wide health‑management initiatives. Provide input into the practice site’s financial, clinical, and service quality performance; budget, capital plan, and personnel issues. Provide clinical input into the development and review of appropriate clinical protocols and processes. Enhance professional growth and development through participation in educational programs, literature review, in‑services, and workshops. Perform other functions as required. Attend and participate in all scheduled meetings, including PCC, provider and site meetings. Modify assessment techniques according to patient condition, culture, age, and stage of development. Maintain coding and encounter data standards for completeness, accuracy, timeliness, and reimbursement level, including legible documentation. Qualifications NewMexico Nurse Practitioner – Masters degree in Nursing; NewMexico Nurse Practitioner license; ANCC or AANP certification required. Minimum of two years experience preferred. Physician Assistant – Baccalaureate prepared; CAHEA Certified Physician Assistant or two (2) years working experience as a Certified Physician Assistant, and licensed by the NewMexico Board of Medical Examiners required.
