Reports To Chief Medical Officer / Administrative Manager About the Role As an Internal Medicine Physician at TRUE HEALTH, you will play a crucial role in providing comprehensive healthcare to adult patients across multiple locations in Hicksville, Brooklyn, and Fresh Meadows. Your primary responsibility will be to diagnose and manage a wide range of medical conditions, ensuring that patients receive personalized and effective treatment plans. You will collaborate with a multidisciplinary team to promote preventive care and manage chronic illnesses, ultimately improving patient outcomes and quality of life. This position requires a commitment to continuous learning and adapting to the evolving healthcare landscape, as well as a dedication to patient‑centered care. Minimum Qualifications Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution. Board certification in Internal Medicine or eligibility for board certification. Valid medical license to practice in New York State. Preferred Qualifications Experience in a primary care setting or outpatient clinic. Familiarity with electronic health record (EHR) systems. Bilingual proficiency in English and Spanish or other languages. Graduated from an accredited medical school and licensed to practice in New York State. Board certified or board eligible status. At least 1‑2 years of experience preferred. Ability to travel to other sites for coverage as needed. Bilingual in English and Spanish or Creole/French required. Responsibilities Conduct thorough patient evaluations, including medical history assessments and physical examinations. Diagnose and treat a variety of acute and chronic medical conditions, developing individualized treatment plans. Order and interpret diagnostic tests, such as lab work and imaging studies, to inform clinical decision‑making. Provide preventive care services, including vaccinations and health screenings, to promote overall wellness. Collaborate with other healthcare professionals to coordinate patient care and ensure comprehensive treatment. Provide comprehensive medical care to patients from adolescent years and older in ambulatory care settings. Provide consultations by examining patients; reviewing medical records, reports, and logs; researching information and developing conclusions and care recommendations. Adhere to professional standards, practice policies and procedures, federal, state, and local requirements, and regulatory standards. Work as a team player assisting other staff providers with developing diagnosis and patient treatment options. Prescribe or administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, or injury. Order, perform, and interpret test and analyze records, reports, and examination information to diagnose patients’ condition. Obtain and maintain hospital privileges at a Hospital Medical Center in order to provide and coordinate care for patients in the hospital setting. Record and maintain patient information such as medical history, reports and examination results. Monitor patients’ conditions and progress and re‑evaluate treatments as necessary. Review health risk assessments (HRAs) to help determine a plan of care. Explain procedures and discuss test results or prescribed treatments with patients. Refer patients to medical specialists or other practitioners when necessary. Participate in LEAN and other activities necessary to implement the multi‑disciplinary patient‑centered medical home approach for the care of pediatric patients. Update job knowledge by participating in continuing medical educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
