With a smaller patient panel, you’ll be able to dedicate the time and attention each participant deserves. What You’ll Do Conduct thorough, in-person physical and wellness assessments. Collaborate with the Interdisciplinary Team to design and implement individualized care plans. Coordinate participant transitions between acute, short-term, and long-term care settings. Focus exclusively on holistic care — no hospital rounds — with an average of eight patients daily. Requirements Completion of an accredited residency program (ACGME required) MD or DO degree Board Certification preferred Active, unrestricted California state medical license State CDS certification and DEA registration Eligibility to participate in Medicare and Medi-Cal programs Specialties considered: Internal Medicine, Family Medicine, Geriatrics and Emergency Medicine 401(k) matching Health insurance Malpractice insurance Paid time off Relocation assistance Vision insurance Ready to Join Us?
