The Opportunity Town Square Health is hiring a Primary Care Provider (MD/DO) in the Rochester area to deliver high-quality primary care in a Medicare‑focused, value‑based care environment. This is a clinic‑based role with the time, team support, and resources to practice comprehensive medicine: patient panels smaller than the national average, 20–40‑minute visit lengths based on patient needs, and a team‑based care model with two Medical Assistants paired with each provider. You’ll play a key role in delivering excellent clinical care, coordinating across the care team, and helping shape a new care model designed to improve outcomes and patient experience.
