Responsibilities Deliver comprehensive primary care at home and senior community visits to patients. Represent Bloom Healthcare in the local market as an ambassador supporting new market growth and conducting market analysis. Strategically plan to grow patient panel and deliver home visits to approximately 10 patients per day. Build trusted relationships by engaging key members of patients’ care teams, including assisted living facilities, hospitals, clinics, and community organizations for care coordination. Collaborate with interdisciplinary teams—specialists, pharmacists, and social workers—to coordinate care, address complex patient needs, and achieve optimal health outcomes. Provide data‑driven, condition‑specific specialty visits to patients as needed. Manage health outcomes of an empaneled patient population. Utilize population health data to drive superior patient outcomes. Attend ongoing educational opportunities. Commute to assigned assisted‑living communities for home care. Occasional travel to Denver for initial and ongoing training. Physician Benefits Scheduled days Monday–Friday (3, 4, or 5 days). Relocation assistance packages available.
