Collaborative Care Model: Join a tight-knit, established team of two daytime physicians and one daytime APP, offering a supportive environment for professional growth. Lead Patient-Centered Care: Diagnose and treat acute and chronic conditions, ensuring every individual receives a clear path toward recovery and long-term wellness. Manage the Clinical Journey: Drive the full continuum of care—from initial assessment and diagnostic interpretation to preventative screenings—to identify health risks early. Create Collaborative Treatment Plans: Coordinate seamlessly with physician partners and a robust network of specialists, ensuring patients never navigate their health journey alone. Empower Patients & Families: Use proactive education and counseling to give patients the tools and confidence to manage their health effectively outside the hospital. Document Excellence: Capture the patient story with precision and care, maintaining high-quality records that serve as a reliable roadmap for the entire care team. About the Facility Ascension Mercy Hospital is a 150‑bed facility in Oshkosh, recognized as a Level III Trauma Center and a Certified Stroke and Chest Pain Center. With a strong 24/7 Hospitalist, Emergency, and ICU presence, our clinicians work in a high-functioning environment supported by a full spectrum of primary and subspecialty care. Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. Nurse Practitioner credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Drug Enforcement Administration credentialed from the Drug Enforcement Administration preferred. Education Master's degree of Nursing required.
