Thrive Here.Additional Job Description: POSITION SUMMARY : The Advanced Practice Professional (APP)has coordination and clinical skills required for the comprehensivemanagement of pediatric surgery patients, ( including biological,social and psychological assessments). The APP coordinatesinvolvement of medical specialty teams necessary to develop acomprehensive plan for the patient, and reviews the plan of carewith the other members of the clinical team, participates indirecting that care, performs diagnostic/therapeutic procedures andcoordinates discharge planning (as applicable) . The APPparticipates in the education of residents and medical/nursingstudents.Provide healthcare services consistent with state law and therequirements of the professional licensing or certificationauthority and/or hospital privileges, where applicable . Suchservice may include, but are not limited to . Other duties may be assigned.1.Manages comprehensive and individualized care for pediatric surgerypatients and their families in the inpatient phase ofhospitalization immediate post discharge and in the ambulatorysetting.2.Completes daily rounds; consults and/orpre-procedure/evaluations.3.Completes integrated assessments.4.Participates in the perioperative management of pediatric surgerypatients which may include, but is not limited to F irst A ssist inthe operative suite.5.With the collaboration of pediatric surgery faculty or by followingestablished clinical Guidelines and Protocols, orders additionaltests or consults as deemed appropriate .6.Performs and communicates results of diagnostic tests followingestablished clinical guidelines and scope of practice.7.Consults and communicates with clinic and hospital staff regardingabnormal findings in the evaluations of patients.8.Performs succinct comprehensive clinical evaluations and documentsfindings in medical record .9.Facilitates patient discharge by coordinating appropriate servicesand tests prior to discharge necessary for outpatient follow-upappointments in the clinic (as applicable) .10.Develops a comprehensive individualized plan of care for pediatricsurgery patients, which includes teaching, discharge planning andnecessary referral to other members of the health care team asnecessary.11.Demonstrates expertise in caring for patients across the agecontinuum of their educational experience and certification.12.Identifies unstable or complex pediatric surgery patient caresituations and facilitates appropriate intervention in the clinicand/or hospital setting.13.Recognizes emergency situations and initiates effective emergencycare.14.Evaluates pediatric surgery patients and serves as a coordinatorfor follow up procedures and tests in the ambulatory setting (asapplicable).15.Assists in the collection of outcome measures (asapplicable).16.Demonstrates critical thinking and diagnostic reasoning skills inclinical decision making by the identification, evaluation andmanagement of the individual needs of patients/families.17.Demonstrates the ability to utilize equipment in a safemanner.18.Bases decisions made/actions taken on the relevant scientificprinciples, established standards of care and/or researchfindings.19.Utilizes teaching/learning theory and appropriate teachingstrategies in the provision of education to nursing staff, hospitalstaff, students, patients, families and the community.20.Promotes an environment that facilitates learning by responding toappropriate patient/family preferences, priorities and readiness tolearn.21.Updates own knowledge base throughout the year.22.Completes all identified clinical competencies.23.Serves as a resource person for staff caring for the pediatricsurgery patient.24.Provides educational programs to professional and paraprofessionalpersonnel in area of expertise.25.Participates in community programs, distribution of literature, asappropriate in area of expertise.26.Documents the plan of care, interventions and evaluations in aclear and concise manner for every patient visit in the patient’smedical record following approved hospital/UHA policies andcompliance standards for documentation .27.Collaborates with other leaders and staff to effectively achievepatient outcome based improvements in care.28.Provides leadership in the development and implantation of changesin the pediatric surgery practice that positively impact patientoutcomes .29.Works collaboratively with medical and nursing staff to facilitatecreative problem solving and provide professional support.30.Participates and initiates inter-disciplinary approaches to patientcare, program development and education .
