Position Summary As an independent Optometrist leasing space inside Walmart stores, you will lead patient care for eye and vision health, maintaining autonomy over practice schedules, fee structures, and patient management. Responsibilities Establish your days of practice, hours of operation, patient schedule, and fee schedule. Maintain 100% of exam and professional fees. Provide eye and vision care and general health services to patients. Optional Services (for Fair Market Value) Opt into equipment and instrumentation. Opt into administrative and/or Opt-Tech support services (if state allows). Minimum Qualifications Doctor of Optometry (OD) from an accredited optometry school with a valid state optometry license. Ophthalmologist (MD or DO) from an accredited school with a valid state ophthalmology license. General Liability Certificate. Professional Liability Insurance Certificate. W-9 form.
