Position Summary Independent Optometrist – Sam’s Club. Launch your services in Sam’s Clubs! As an Independent Optometrist leasing space inside our Sam’s Club, you will take a leading role in patient care with respect to eye and vision care as well as general health and well-being. What You’ll Do Establish your days of practice, hours of operation, and patient schedule. Set your fee schedule and keep 100% of exam/professional fees. Optionally, opt into equipment/instrumentation and administrative/Opt-Tech support services (if state allows). Minimum Qualifications Doctor of Optometry (OD) from an accredited optometry school, with a valid state optometry license. Ophthalmologist (MD or DO) from an accredited school, with a valid state ophthalmology license. General Liability Certificate. Professional Liability Insurance Certificate. Valid W‑9 form.
