The director balances 80% clinical time and 20% administrative time, practicing primary care alongside provider colleagues. Key responsibilities include strategic leadership, provider onboarding, coaching, and driving organizational initiatives that support clinical excellence and growth. Core Responsibilities Support Organizational StrategyCollaborate with Practice Manager, Outreach Manager/Director, and regional leadership to implement strategic initiatives. Execute OSH objectives—Best Care Anywhere, Unmatched Patient Experience, Exceptional Growth, Great Place to Work, and DE&I leadership. Communicate organizational information to care teams and relay concerns upward. Provider ManagementCoordinate and lead onboarding of new providers. Model and implement the care model, ensuring rigorous daily huddles, patient care planning, admission prevention, and documentation workflows. Guarantee evidence‑based care and adherence to Oak Street clinical guidelines. Support preventive care initiatives and the value‑based care model. Address root causes when metric goals are unmet. Leadership and CoachingCreate a culture of growth, coaching, and coaching through regular feedback and performance reviews. Build constructive problem‑solving processes. Celebrate successes and communicate opportunities for improvement. CultureMaintain a positive, joyful environment aligned with Oak Street values. Advocate triad best practices. Ideal Candidates MD, DO, APN, or PA – Board‑certified in Internal Medicine, Family Medicine, or relevant specialties. Experience in clinical leadership and provider coaching. Proficiency in metrics‑driven care and quality improvement projects. Passion for helping older adults and low‑income patients. Comfort with ambiguity, problem solving, and calmness under pressure. 5+ years in outpatient practice (preferred). Fellowship in Geriatrics, advanced degrees (MBA, JD, MPH), or executive experience welcomed. US work authorization required. Pay Range Typical annual salary: $174,070.00 – $374,920.00.
