Responsibilities Accountable for outcomes, providing strong clinical care and cost‑effective management for a panel of up to 450 patients. Act as a health coach, helping patients set and achieve short‑ and long‑term health goals and following up on progress. Simplify and prioritize care to make behavior change actionable for patients and to lead care teams toward performance goals. Measure and meet metrics such as patient admissions per thousand, appointment slot usage, CGCAHPS, clinical gap closures, and medical cost measures. Provide independent assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for acute and complex chronic illnesses. Engage with hospitalists for in‑hospital patients and coordinate care with transitional care teams for patients in hospital, SNF, LTC or homebound. Lead a care team consisting of a medical assistant, care facilitator and care coordinator. Cover for other providers and assist in recruiting patients and additional providers for the center. Supervise and collaborate with advanced practice practitioners, remaining accountable for their actions. Perform other duties as assigned and modified at the manager’s discretion. Knowledge, Skills & Abilities Availability and accessibility: open to walk‑in visits and timely response to calls and messages. Service orientation: provide care that would be given to a family member at every interaction. Evidence‑based medicine: stay updated on evidence‑based practices, lifestyle, nutrition and preventive care. Leadership: continuously develop leadership skills for patients, team, center and company. Quality focus: work with care teams to achieve high‑quality outcomes and continuously improve practice. Influence: compassionately influence patients, teams and self to achieve best outcomes. Self‑care: manage personal physical, mental, social and spiritual health to sustain physician wellness. Proficiency in Microsoft Office Suite and related software. Travel willingness: local travel as needed, national travel for initial training and occasional regional or national recruitment or training. Fluency in English; additional language fluency may be required at application time. Independent judgment in use and exercise. Education and Experience Criteria MD or DO in Internal Medicine, Family Medicine, Geriatrics or a similar specialty. Ability to obtain or already hold an active State Medical License for the state(s) of practice. Board certification or eligibility in the specialty, and ongoing maintenance of certification through MOC, CME or retesting. Current DEA number for Schedule II‑V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within the first 90 days of employment. Compensation Pay range: $221,141 – $315,915 per year. Final compensation depends on experience, education, geographic location and other factors. Eligibility for bonuses or commissions is also possible.
