Overview ChenMed is a physician‑led primary care organization that focuses on providing high‑quality, value‑based care to seniors. We are expanding rapidly and seek dedicated primary care physicians to join our team as Partners. Responsibilities Demonstrate accountability for outcomes and cost‑effectiveness for a risk‑adjusted panel of at least 400 patients. Act as a health coach, helping patients set short‑ and long‑term health goals and providing frequent follow‑up. Simplify and prioritize care plans so that behavior change is actionable for patients and care teams. Meet performance metrics such as patient admissions per thousand, appointment slot utilization, CGCAHPS scores, clinical gap closures, and medical cost measures. Collaborate with hospitalists, emergency medicine, and other specialists to ensure seamless care transitions. Lead a care team consisting of a care promoter, care facilitator, and care coordinator for in‑office patients; coordinate with transitional care teams for homebound, SNF, LTC, or in‑hospital patients. Serve as a role model for culture, leading positive attitudes of love, accountability, and passion within the center. Work closely with managers and medical directors on guidelines, risk management, quality programs, and regulatory compliance. Essential Job Duties / Responsibilities Provide independent care for acute and chronic illnesses in older adults, taking full accountability for patient outcomes. Engage with hospitalists and other providers when patients are hospitalized. Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document for complex chronic patients. Cover for other providers as needed and assist in recruiting patients and additional providers. Supervise and collaborate with an Advanced Practice Practitioner (APP) and remain accountable for the APP’s actions. Perform additional duties as assigned by management. Knowledge, Skills and Abilities Availability for patient visits, walk‑ins, calls, and timely messages. Service orientation – providing care that one would give a family member. Evidence‑based medicine, with continuous updates on clinical, nutritional, and lifestyle interventions. Leadership development to benefit patients, teams, and the organization. Commitment to quality and continuous improvement of practice operations. Influence patients, teams, and oneself to achieve optimal outcomes. Self‑care to maintain personal well‑being and physician wellness. Proficiency in Microsoft Office Suite and other common office software. Willingness to travel locally, regionally, and nationally for training and recruitment. Fluency in English; additional language fluency may be required for certain centers. Education and Experience MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a comparable specialty. Current, active MD licensure in the state of employment. Minimum six years of clinical experience in geriatric, adult, or family practice, including at least two years of experience as a ChenMed primary care partner. Board certification or board‑eligible status in the terminal specialty; maintaining certification through MOC/CME or re‑examination. Current DEA number for scheduleII‑V controlled substances. Basic Life Support (BLS) certification within the first 90 days of employment. Pay Range $214,700 – $306,714 per year.
