Position Summary Launch your services in Walmart stores! As an independent Optometrist leasing space inside our Walmart stores, you will take a leading role in patient care with respect to eye and vision care as well as general health and well‑being. Responsibilities Establish your days of practice, hours of operation, patient schedule, and fee schedule, keeping 100% of exam/professional fees. Control your patient schedule and fee structure. Optionally opt into equipment/instrumentation and administrative/Opt‑Tech support services if state allows. Manage lease agreements through Gumberg Asset Management Company. Minimum Qualifications Doctor of Optometry (OD) from an accredited optometry school. Valid state optometry license. Or Ophthalmologist (MD or DO) from an accredited school. Valid state ophthalmology license. General Liability Certificate. Professional Liability Insurance Certificate. W‑9 form. Preferred Qualifications Experience in independent leasing or ownership of optometry practices. Knowledge of state regulations regarding independent lease holders.
