Primary Care Physician Responsibilities Accountability for outcomes: Demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for up to 450 patients. Coaching for health: Acts as a health coach, helping patients set short and long‑term health goals, partnering with patients to work toward those goals, and following up regularly. Simplifying for action: Simplifies and prioritizes appropriately to make behavior change actionable for patients and for leadership of care teams. Performance Metrics Metrics regularly measured for each PCP’s panel include patient admissions per thousand, utilization of 18‑21 appointment slots per day (new patients count for 2 slots, follow‑ups for 1 slot), CGCAHPS (patient experience), clinical gap closures, and medical cost measures. Each PCP will have goal targets and is expected to work toward those with center and market leadership and care teams. Culture and Leadership PCPs are expected to champion a positive culture of love, accountability, and passion, working both independently and collaboratively with other health‑care professionals. They will participate in risk and quality management programs, clinical meetings, and other initiatives that promote patient health and company goals. Essential Job Duties & Responsibilities Independently provide care for patients with acute and chronic illnesses in the older adult population. Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining responsible for the detailed care of the patient. Engage with the hospitalist whenever a PCP’s patient is hospitalized, regardless of hospital affiliation. Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document for patients with acute and complex chronic needs. Lead a care team that includes a medical assistant, care facilitator, and care coordinator for office‑capable patients. Coordinate with the transitional care team and other resources for patients unable to attend office visits (in‑hospital, SNF, LTC, or homebound). Actively participate in center management, cover for other providers as needed, and assist in recruiting patients and additional providers for the company. Supervise, collaborate with, or function within a practice or collaborative agreement with an Advanced Practice Practitioner (APP), remaining accountable for the APP’s actions when employed with the company. Perform other duties as assigned and modified by management discretion. Knowledge, Skills & Abilities Availability and accessibility to build trust with patients. Service orientation: provide care that one would want for a family member or oneself. Evidence‑based medicine: remain updated on clinical, nutritional, and lifestyle‑based interventions. Influence: competently and compassionately influence patients, teams, and oneself to achieve optimal outcomes. Self‑care: maintain personal physical, mental, social, and spiritual health to care for patients effectively. Proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook). Willingness to travel locally as needed, nationally for initial training, and occasionally regionally and nationally for recruiting or training. Fluency in English (verbal and written). Flexibility to work evenings, weekends, and holidays as required. Use of independent judgment and flexibility in work schedule. Education & Experience Criteria MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty required. Must obtain or already hold an active state medical license for the state(s) of practice. Board certification in the specialty is preferred; board eligibility required. Maintain board certification through MOC, CME, or retesting as required. Current DEA number for schedule II‑V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within the first 90 days of employment. Pay Range $214,700 – $306,714 salary. #J-18808-Ljbffr
