ESSENTIAL JOB DUTIES/RESPONSIBILITIES Provide independent primary care to patients with acute and chronic illnesses among older adults. Take full accountability for patient care and outcomes, including assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation. Lead a care team that includes a care promoter (medical assistant), care facilitator, and care coordinator for patients able to visit the office. Engage with hospitalists and transitional care teams for patients who are hospitalized, in skilled nursing facilities, long‑term care, or homebound. Actively participate in center management, including covering for other providers and providing leadership within the organization. Recruit patients for the center and support the hiring of additional providers as needed. Perform additional duties as assigned by management. CORE ATTRIBUTES Accountability for outcomes: deliver strong clinical care and cost‑effective management for a panel of up to 450 patients. Health coaching: help patients set short‑ and long‑term health goals and support behavior change. Simplify and prioritize: make behavior change actionable for patients and guide care teams toward performance goals. KNOWLEDGE, SKILLS & ABILITIES Availability: keep patients able to access care through walk‑ins and timely communication. Service orientation and evidence‑based medicine: provide quality care and stay current with clinical, nutritional, and lifestyle interventions. Leadership: work to develop and improve leadership skills to benefit patients, teams, and the organization. Quality focus: collaborate with care teams to achieve high quality outcomes and continuous improvement. Influence: competently and compassionately influence patients and teams. Self‑care: maintain physical, mental, social, and spiritual health to support practice sustainability. Computer proficiency: use Microsoft Office Suite and related software. Travel: willingness to travel locally, regionally, or nationally for training and recruiting as required. Language: fluency in English; other languages may be required depending on the site. EDUCATION AND EXPERIENCE CRITERIA MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Current State Medical License for the state(s) of practice. Board certification preferred; board eligibility required. Maintenance of certification through MOC, CME, or retakes. Current DEA number for ScheduleII–V controlled substances. Basic Life Support (BLS) certification within the first 90 days of employment. PAY RANGE $221,141 – $315,915 (base salary or hourly rate). #J-18808-Ljbffr
