Job Description Summary Advanced Practice Provider (Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA)) responsible for total care of patients in collaboration with the physician of record and the patient care team. Eligible for a $10,000 Sign‑On Bonus and must hold a Florida license in good standing. Essential Duties and Responsibilities Deliver patient-centered, evidence‑based care for acute, chronic, and palliative needs. Perform patient assessment and examination, develop care plans, and initiate interventions as indicated. Diagnose and decide on treatment; analyze x‑rays and lab results; write prescriptions; order imaging, therapies, and durable medical equipment. Provide surgical assistance when required. Offer instruction and assistance on symptom management and advanced care planning for patients with chronic life‑limiting illness. Assist in meeting outstanding Quality Measures and Hierarchical Condition Category (HCC) goals. Maintain ongoing communication with the primary care provider to ensure continuity and coordination of care, and consult with the supervising physician on complicated diagnoses or treatment decisions. Collaborate with regional palliative/hospice agencies to facilitate end‑of‑life care and services when appropriate. Complete all documentation and billing in a timely manner per protocol. Identify patients who would benefit from skilled nursing/therapy and initiate appropriate processes for qualifying value‑based patients. Ensure age‑appropriate care for an adult population and communicate health information effectively to patients, families, and team members. Participate in interdisciplinary projects to improve organizational performance and outcomes. Compassion and Clinical Competence Exhibit strong clinical skills to improve patient outcomes. Qualifications Master’s degree in nursing or equivalent education/experience; two or more years of related experience preferred. Current Florida license as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA). CPR/BLS certification preferred. Proficiency in reading complex documents and communicating effectively. Strong mathematical and reasoning abilities to interpret data and make decisions. Computer literate: experience with Athena, Microsoft Excel, Word, and learning additional programs as required. Physical Demands and Work Environment Regular standing, walking, use of hands, and occasional light lifting up to 10 pounds. Requires ability to sit for extended periods.
