Key Responsibilities include: Diagnose and treat a broad range of acute and chronic conditions, conduct physical exams, order and interpret diagnostic tests, and develop and implement individualized treatment plans. Leverage current knowledge and recent experience in providing direct patient care. Participate in team-based care by joining daily huddles within the Patient Aligned Care Team - comprised of an RN care manager, LPN, clinical pharmacist, psychologists, and social workers. Collaborate closely with other co-located extended team members, such as clinical pharmacists and nutritionists to ensure coordinated, wraparound care. Contribute to team and facility-wide quality improvement, population-based interventions, leadership initiatives, research, clinical projects, educational activities, and/or program development in areas that include but are not limited to chronic disease management, care transitions, geriatrics, reproductive and women's health care, and LGBTQ+ care. Requires: U.S. Citizenship, an M.D. or D.O. Full, active, and unrestricted license to practice medicine in any state Board C certification or eligibility in Internal Medicine or Family Medicine is preferred but not required Passion for teaching and mentoring others in the Primary Care setting Education Debt Reduction Program (EDRP) Eligible: EDRP is a student loan reimbursement program valuated at up to $200,000.
