Case Log Requirements: Either documentation of Case Logs for past 2 years or Activity Verification Form completed within hospital application. Facility Procedure Requirements: Must be comfortable with all requested privileges on delineation of privileges (DOP). Malpractice Coverage Requirements: $1M/$3M.
