Responsibilities The PCP independently provides care for patients with acute and chronic illnesses encountered in older adults. The PCP will take full accountability for patient care and outcomes, appropriately seeking consultation from specialists when needed while remaining responsible for the patient’s detailed care. The PCP will engage with the hospitalist whenever a patient is hospitalized, regardless of the hospitalist’s affiliation. The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs. The PCP leads a care team composed of a care promoter, a care facilitator, and a care coordinator for patients who can attend the office; for patients who cannot, the PCP engages transition‑care teams, case managers, and other resources as appropriate to the market. The PCP has an active role in center management and helps cover for other providers when necessary. The PCP takes an active role in recruiting patients for the center and recruits additional providers for the company as required. Performs other duties as assigned and modified at the manager’s discretion. This job requires the use and exercise of independent judgment. Knowledge, Skills & Abilities Availability and Accessibility: Build trust by being open and available for walk‑in visits and responding to phone calls and messages promptly. Service Orientation: Provide care that you would want for a family member or yourself to every patient at each interaction. Evidence‑Based Medicine: Stay updated on evidence‑based medicine, lifestyle, and nutrition interventions to improve patient outcomes. Physician Leadership: Continuously develop and improve leadership skills for the benefit of patients, team, center, and company. Quality: Work with care teams to achieve high‑quality outcomes and quality measures, seeking systematic improvements in practice and operations. Influence: Competently and compassionately influence patients, teams, and yourself to achieve the best outcomes. Self‑Care: Maintain physical, mental, social, and spiritual well‑being to provide the best care for patients. Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other related software used within the company. Willingness to travel locally as needed, nationally for initial training (2 weeks), and occasionally regionally and nationally for recruiting or training purposes. Fluency in English (verbal and written). Other languages may be required as specified at application. Education and Experience MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty is required. Must be able to obtain a State Medical License or already hold an active State Medical License for the state(s) in which the position will be located. Board certification in the specialty preferred; board eligibility required. Once certified, maintain certification through MOC, CME, or retaking exams as required. Must have a current DEA number for schedule II–V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within the first 90 days of employment. Pay Range $221,141 – $315,915 Salary. The posted pay range represents the base hourly rate or base annual full‑time salary for this position. Final compensation will depend on experience, education, geographic location, and other relevant factors. The position may also be eligible for bonuses or commissions.
