ESSENTIAL JOB DUTIES/RESPONSIBILITIES Independently provide care for patients with acute and chronic illnesses among older adults. Takes full accountability for patient care and outcomes, seeking consultation from specialists as needed. Upon mastery of the Core Model, play an active role in center management, supervise physicians, and cover for other providers when necessary. Engage as needed in recruiting patients and additional providers for the company. Participate in facilitated practice and leadership rounding to support the growth of other providers. Collaborate with hospitalists for patients in hospital settings. Manage assessment, diagnosis, treatment, education, health promotion, care coordination, and documentation for patients with acute and complex chronic needs. Lead a care team consisting of a medical assistant, care facilitator, and care coordinator for patients attending office visits. Cooperate with transition‑care teams, case managers, and other resources for patients not able to attend office visits. Supervise and collaborate with an Advanced Practice Practitioner, remaining accountable for their actions. Demonstrate mastery of the Core Model before progressing to accelerated leadership development. Participate in leadership development programs and take on greater leadership roles within the center. Assist in the selection, training, onboarding, and coaching of new providers. Lead weekly meetings (Super Huddle and Transforming Care Meeting) and support the center operations director when needed. Fill in as needed for clinical leadership responsibilities and monitor daily center census. Assist center and market leadership in planning actionable clinical outcomes and provide coaching to other providers. Perform other duties as assigned or modified by management. KNOWLEDGE, SKILLS AND ABILITIES Advanced‑level business acumen with expert knowledge of core functions, practices, and processes. Proficiency in Microsoft Office Suite and other company applications. Willingness to travel locally, regionally, and nationally up to 10% of the time; flexibility for evening, weekend, and holiday work. Openness to commuting to neighboring centers and relocating to other markets if desired. Demonstrate emotional intelligence and respond appropriately to others in all interactions. Communicate effectively by listening actively and adapting style to different audiences. Build trust and credibility through consistent, reliable actions that demonstrate integrity and respect. Provide specific feedback and coaching to support growth and improvement. Manage time effectively by prioritizing, meeting deadlines, and delegating when appropriate. Fluency in English; other languages required for specific centers will be disclosed during application. Use independent judgment in all aspects of the role. EDUCATION AND EXPERIENCE CRITERIA MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty. Current active MD licensure in the state of employment. Minimum of 1 year of clinical experience in a geriatric, adult, or family practice setting preferred. Prior leadership experience (e.g., chief resident, geriatric fellowship) preferred. Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty; board eligibility required. Maintain board certification through MOC, CME, or reexamination as required. Current DEA number for scheduleII‑V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross within 90 days of employment. PAY RANGE $221,141 - $315,915 (Base salary; final compensation depends on factors such as experience, education, and location. Position may be eligible for bonuses or commissions.)
