Job Summary The Vice President, System Care Management, reports to the Senior Vice President, Care Transitions & Integration, and is responsible for leadership and oversight of Care Management functions across Harris Health System. This role will serve as strategic change activator in care management, guiding the efforts of the Administrative Directors, and will provide leadership for the multidisciplinary inpatient, utilization review, ambulatory, community-based, and outsourced care management teams, including nurse case managers, utilization review nurses, social workers, community health workers, and care management coordinators. The responsibilities of this role include, but are not limited to: Developing a Care Management strategic plan and supporting Harris Health System's efforts to become a high reliability organization, including the inpatient, utilization review, ambulatory and outsourced platforms. Ensuring consistency of policies, procedures, and practice; monitoring compliance with regulations and ensuring consistent application of regulatory changes at all entities. Monitoring key performance metrics that support the practice of Care Management; supporting the triple aim, including improving patient outcomes and satisfaction. Working collaboratively with Harris Health System's inpatient, utilization review, ambulatory, and outsourced services to create an integrated comprehensive care management infrastructure that ensures appropriate utilization and seamless transitions of care. Developing and managing the organization’s Care Management program and ensuring consistent understanding and application. Establishing clear educational goals and a training plan for Care Management and working with the Director, Care Management Education and Coaching, to implement. Establishing competencies for each position in Care Management and a standardized process for evaluating competencies. Collaborating with system- and community-wide stakeholders to align the Care Management program with the organization’s strategic plan. Guiding innovative care management efforts to remain competitive with peer organizations. Minimum Qualifications Degrees: Graduation from an accredited School of Nursing with a Bachelor’s degree in Nursing and a Master’s degree in Nursing or related field (Healthcare Administration or Business Administration) required. License & Certification: Licensed to practice nursing in the State of Texas required. Accredited Case Manager certification (ACM or CCM) to be completed within two (2) years if not held at the time of appointment. Work Experience: Fifteen (15) years of experience in a healthcare environment required. Medical/Surgical/ICU with hospital leadership experience in a complex health system managing multiple departments or facilities preferred. Specific experience in care or case management in acute care and ambulatory settings to include care coordination, utilization review, utilization management, transition planning, and social work. Exceptional leadership, communication, relationship building, and public speaking skills as well as a functional knowledge of the CMS Conditions of Participation, Joint Commission standards, and Case Management Standards of Practice and Scope of Services is required. Experience in a safety net and/or academic healthcare system is preferred. Management Experience: Ten (10) years of leadership or supervisory experience in complex health systems required. Management of multiple departments or facilities preferred. Communication Skills: Above Average Verbal (Heavy Public Contact), Exceptional Verbal (Public Speaking), Writing/Correspondence, Writing/Reports. Proficiencies: MS Excel, MS Word, PC.
