As a Mobile Primary Care Physician with Twin Cities Physicians, you'll focus on identifying risks early, managing medically complex patients, and helping seniors remain healthier within the communities they call home. You’ll work across assisted living, memory care, skilled nursing, and long-term care environments, helping prevent avoidable hospitalizations while improving quality of life for a highly vulnerable patient population. Responsibilities Provide on-site primary care and geriatric rounding throughout assisted living, memory care, skilled nursing, and long-term care communities. Manage chronic conditions, transitional care needs, preventative care initiatives, and medically complex patient populations. Identify clinical concerns early and implement interventions that improve outcomes. Reduce hospitalizations and emergency room utilization through proactive care delivery. Perform diagnostics, treatment planning, medication management, and follow-up care. Collaborate with Nurse Practitioners, Physician Assistants, nursing staff, therapists, and case managers. Maintain accurate EMR documentation, care coordination, and ongoing clinical oversight. Qualifications Active Minnesota Physician (MD or DO) license in good standing. Experience in geriatrics, internal medicine, family medicine, hospital medicine, post-acute care, or related specialties preferred. Familiarity with chronic disease management, transitional care, or value-based care models is beneficial. Strong clinical judgment and confidence managing medically complex patient populations.
