Practice Location Primary Office Qualifications Board Certified or Board Eligible in Endodontics Endodontic Residency Certificate required Graduate from an accredited dental school Current Washington State Dental License (or ability to obtain prior to employment) Current CPR, DEA, and NPI certifications Minimum of 1 year of experience preferred Education Doctor of Dental Surgery or better. High School or better.
