About the Role The Primary Care Physician (PCP) will work with a panel of up to 450 patients in a value‑based primary care setting, focusing on managing acute and chronic illnesses in older adults, promoting behavior change, and coordinating care access. Key Responsibilities Accountability for outcomes – provide strong clinical care and cost‑effective management for each patient. Health coaching – help patients set and achieve short‑ and long‑term health goals with frequent follow‑ups. Simplify and prioritize care plans so that behavior change is actionable for patients and staff. Provide independent care for acute and complex chronic illnesses, while consulting specialists as needed. Engage with hospitalists during inpatient stays and coordinate transitions with hospital, SNF, LTC, or homebound teams. Document assessment, diagnosis, treatment, health promotion, education, and care coordination. Lead the care team consisting of a care promoter, care facilitator, and care coordinator for in‑office patients. Assist with center management, cover for other providers, and recruit new patients and providers as required. Supervise and collaborate with Advanced Practice Practitioners (APPs) and remain accountable for their actions. Perform additional duties as assigned by management. Proficiency with Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other common office software. Willingness to travel locally, nationally for training, recruiting, or regional purposes as needed. Fluent English; additional language fluency where required. Required Credentials MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty. Current state medical license (active) in the state(s) of practice. Board certification or eligibility in the relevant specialty. Current DEA number for Schedule II‑V controlled substances. Basic Life Support (BLS) certification within 90 days of employment.
