Primary Care Physician-2 at ChenMed, Jacksonville, FL. ESSENTIAL JOB DUTIES/RESPONSIBILITIES Independently provides care for patients with acute and chronic illnesses encountered in older adults. Takes full accountability for patient care and outcomes, seeking consultation from specialists when needed while remaining responsible for detailed patient care. Engages with hospitalists regarding patients admitted to hospitals (regardless of the hospitalist’s affiliation). Conducts assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs. Leads a care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients who can visit the office. For patients unable to come to the office (in hospital, SNF, LTC, or homebound), engages with the transitional care team, case managers, acute and transitional‑care physicians, and other resources available in the market. Participates actively in center management, covers for other providers, and helps recruit patients and additional providers as needed. Supervises, collaborates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the APP’s actions while employed with the company. Performs other duties as assigned and modified at manager’s discretion. KNOWLEDGE, SKILLS & ABILITIES Availability and accessibility to build trust with patients, including walk‑in visits and timely phone/message response. Evidence‑based medicine and updated knowledge of clinical, nutritional, and lifestyle interventions. Strong service orientation and quality focus, aiming for high‑quality outcomes and continuous practice improvement. Leadership competency and willingness to develop skills for patients, teams, center, and company success. Compassionate influence over patients, teams, and self to achieve best outcomes. Fluency in English (verbal and written); additional language fluency may be required when indicated. EDUCATION AND EXPERIENCE CRITERIA MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required. State medical license (current or obtaining) for the state(s) of employment. Board certification in the specialty preferred; board eligibility required. Ongoing maintenance of board certification via MOC, CME, or retaking exams as required. Current DEA number for schedule II‑V controlled substances. Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days. Independent judgment ability, travel locally for market needs, and occasional regional/national travel for training or recruiting.
