Family Medicine – Wyoming, MI – PA/NP University of Michigan Health-West is seeking a full‑time, board‑eligible/board‑certified Advanced Practice Provider to join our well‑established and comprehensive primary care team. High caliber, quality focused, innovative providers should apply to expand and provide care to local communities. Provider will be performing a full spectrum of outpatient primary care services for patients, including primary, acute, chronic, and specialty care to patients of all ages and backgrounds. Advanced Practice Providers are responsible for evaluating patients, diagnosing, and treating health conditions with an added emphasis on disease prevention and health management. Candidate will also need to coordinate and collaborate with a multidisciplinary team to ensure a positive experience for all patients, families, and staff. Responsibilities Evaluate, diagnose, and treat a full spectrum of primary care services for patients of all ages. Emphasize disease prevention, health management, and chronic care management. Coordinate and collaborate with multidisciplinary team members. Document care in EMR and engage with virtual care when appropriate. Provide care during extended hours, including occasional weekends. Qualifications Board‑eligible or board‑certified Advanced Practice Provider (PA/NP). Experience in outpatient primary care, with emphasis on acute, chronic, and specialty care. Strong communication skills and ability to work with culturally diverse patients. Fluency in English; additional language skills (Spanish, Vietnamese, Nepali, Bosnian) are an advantage.
