Primary Care Physician – Responsibilities Provide comprehensive care for older adult patients with acute and chronic illnesses. Maintain full accountability for patient care outcomes and seek specialist consultation when necessary. Coordinate care with hospitalists during inpatient stays. Conduct assessments, diagnoses, treatments, and health education, and document all patient encounters. Lead a care team comprising a care promoter, a care facilitator, and a care coordinator. Engage transitional care teams for patients who cannot come to the office (hospital, SNF, LTC, homebound). Actively participate in center management, covering for other providers as needed. Recruit patients for the center and additional providers. Supervise and collaborate with an Advanced Practice Practitioner (APP), remaining accountable for the APP’s actions. Perform other duties as assigned by management. Knowledge, Skills & Abilities Ensure availability and accessibility to build patient trust (walk‑in visits, timely phone and message responses). Provide a service orientation that mirrors care one would give to a family member. Apply evidence‑based medicine while incorporating lifestyle and nutritional factors. Develop and improve physician leadership skills for patients, team, center, and company. Commit to high‑quality care and continuous improvement of practice and operations. Influence patients, teams, and self with compassion and competence. Practice self‑care to maintain physical, mental, social, and spiritual well‑being. Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other common software. Possess willingness to travel locally, regionally, or nationally for training and recruitment. Fluent in English (verbal and written); proficiency in additional languages may be required at certain centers. Exercise independent judgment in clinical and administrative decisions.
