About the Role The Medical Director will deliver comprehensive, high‑quality, person‑centered care across the healthcare continuum, provide clinical leadership and oversight of program team members, and serve as an active medical provider. Work Mode Field based in San Gabriel Valley area. Responsibilities LeadershipDirectly manage a multidisciplinary team of nurse practitioners and physicians to ensure coordinated, high‑quality care delivery. Track and analyze clinical performance metrics related to health outcomes and utilization rates; use data to identify improvement areas and implement strategies. Participate in community stakeholder meetings and address care‑related issues for residents. Provide education and guidance to clinical team members on geriatrics, complex care, and palliative care. Actively participate in provider meetings and quality‑improvement initiatives aimed at improving patient outcomes. Patient CareProvide primary medical care services to patients residing in their place of residence. Collaborate with program nurse practitioners and other interdisciplinary team members to manage complex patients by conducting in‑person and virtual patient rounds. Participate in a telephonic on‑call program to support team members after hours and weekends. Provide immediate support to team members for urgent care needs, such as changes in medical condition or goals‑of‑care discussions. Documentation and ComplianceMaintain accurate and up‑to‑date electronic medical records (EMRs) and other relevant documentation. Adhere to regulatory requirements, coding/documentation standards, guidelines, and quality measures set forth by Medicare and other relevant authorities. Other duties as assigned, including supporting the achievement of SCAN’s vision and goals. Qualifications Doctoral Degree M.D. required. Unrestricted State Medical License required. 5+ years of related experience as a physician, preferably with home‑based visits, geriatrics, chronic disease management, hospice, and/or palliative care. Experience with virtual care/telehealth preferred. Knowledge of the treatment of complex chronic diseases such as dementia, diabetes, heart failure, and COPD. Ability to identify team needs and coordinate with other resources/programs appropriately. Knowledge of HCC and ICD‑10 coding and documentation. Knowledge of wound care and management. Knowledge of CMS guidelines and Medicare Advantage Managed Care. Comfort with initiating goals‑of‑care and end‑of‑life discussions. Strong interpersonal skills and change‑agent mindset to build relationships with healthcare team and patients. Cultural competence and sensitivity required; multilingual skills preferred. Knowledge of and ability to maintain HIPAA requirements. Adaptable and open to change and problem‑solving. Basic technological skills (Word, Excel, PowerPoint, Outlook, EMR) and openness to new technology. Compensation & Benefits Base salary range: $255,000.00 to $367,025.00 annually.
