Primary Care Physician – Conviva Senior Primary Care Location: Deerfield Beach, FL. Summary The Primary Care Physician (PCP) leads a team to provide value‑based care focused on quality for seniors. The PCP collaborates with doctors, advanced practice professionals, pharmacists, nurses, behavioral health, specialists, coders, and referral coordinators to deliver a comprehensive patient experience. Key Responsibilities Evaluate and treat patients in accordance with standards of care. Follow level‑of‑care and quality protocols, monitor care using data and chart reviews. Coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care. Act as an active participant and key source of medical expertise with the care team through daily huddles. Assist Regional Medical Director and Center Administrator in setting a tone of cooperation by displaying a professional and approachable demeanor. Complete all medical‑record documentation timely, working with a quality‑based coder to optimize coding specificity. Follow policy and protocol defined by Clinical Leadership. Meet with RMD to review quality of care, outcome data, policy, procedure and records issues. Participate in potential growth opportunities for new or existing services within the Center. Participate in the local primary care “on‑call” program as needed. Ensure personal compliance with licensing, certification, and accrediting bodies. Allocate 100% of time to direct patient care, including charting and meetings related to patient care. Required Qualifications Current and unrestricted medical license (or willing to obtain one) in the state of practice; eligible to obtain licenses in other states if required. Graduate of an accredited MD or DO program. Excellent verbal and written communication skills. High level of interpersonal skills with colleagues and patients. Fully engaged in the “Integrated team‑based care” model. Willingness and ability to learn and adapt in a value‑based care setting. Superior patient/customer service orientation. Basic computer skills, including email and EMR. Required to participate in the Tuberculosis screening program and be screened when selected. Preferred Qualifications Board certification or eligibility for certification (ABMS or AOA) in Family Medicine, Internal Medicine or Geriatric Medicine. Active and unrestricted DEA license. Medicare Provider Number. Medicaid Provider Number. Minimum of two to five years of directly applicable experience. Experience managing a Medicare Advantage panel with an understanding of best practice in coordinated care environments. Knowledge of Medicare guidelines and coverage. Bilingual ability (plus). Knowledge of HEDIS quality indicators. Compensation & Benefits Scheduled Weekly Hours : 40 Pay Range : $203,400 – $299,500 per year (starting base pay; varies by location and qualifications). Eligible for a bonus incentive plan.
