Responsibilities Establish days of practice and hours of operation. Control patient schedule and set fee schedule. Maintain 100% of exam/professional fees. Optional Options for Fair Market Value Equipment instrumentation. Administrative and/or Opt-Tech support services (if state allows). Minimum Qualifications for lease agreement Doctor of Optometry (OD) from an accredited optometry school. Valid state optometry license. Ophthalmologist MD or DO from an accredited school.
